T-Dap Clinic at Chesterfield County Health Department in Chesterfield on September 17 - 8:30am until 3:30pm. Cost $13 without Medicaid. If you have Medicaid coverage, Medicaid will be billed. Please call the health department to schedule an appointment (843) 623-2117. Students who have not submitted a copy of the shot record by Friday, September 19, will be subject to suspension as per State law.
